  • Is It OSA?

    Hello Everyone,
    I have been to my GP with all the symptoms and a rating of 14 on the initial quiz. I was referred to the sleep clinic but got the appointment of 18th May and thats for only handing me out the testing strip or something. I am really really suffering and my job is on stake now. Being a software engineer sitting all day in front of my computer, i feel embarrased in the office because i kind of go into sleep many times during the day. I have a confusion though. As my wife told me, i stop breathing and wake up every 20 to 30 seconds during the night; my body goes really cold; i sweat a lot;wake up with headaches; tired all the time and the usual OSA symptoms. But when i do remember about waking up, i remember that i struggle while breathing out instead of breathing in. Something behind my nose blocks my breath exit. This causes the lungs to strain. I sometimes, very occassionally though, wake up in a situation where my mind wakes up but my body keeps sleeping. So my mind keeps struggling attempting to wake and move my body. During this i remember that i am mostly struggling with breathing out. I do feel that thing behind my nose while awake as well such as when eating or drinking but my doctor decided not to bother about it. Usual NHS culture isn't it?

    GPs keep saying that you need to loose weight to get rid of this problem but you cant loose weight with this problem because it not only affects the body, it also reduces your ability and willingness to do exercise. I goto gym and play squash everyday but my weight just keeps going up!

    Sorry for the long description and i would appreciate if someone could reply with useful information.

    Most of what you describe sure sounds like it but the exhalation part is puzzling indeed. Can you breathe through your nose OK? What is your BMI exactly? If severely overweight, perhaps your lungs are crushed by the weight around it.

    Shame about the 6 week wait. If you are risking your job and want it looked at faster, you could consider a private sleep study.

    Either way, it's important to get it sorted. Your job is one thing, your health is also at risk, not to mention other poeple's lives if you drive or your work affects other people.

    You are right about losing the weigth, it's extremely hard until you get the OSA under control and get re-energized. If you hammer them a bit, maybe they can move you forward. 6 more weeks can do a lot of damage.

      Thanks for your reply. Yes i was worried about my exhalation problem because after reading this forum for so long, my understanding is that the problem most people have is in inhaling. I am extremely obese with 152KG weight but i am physical not as bad. I mean i have been doing regular exercise, playing cricket, and i play squash for hours. I have good stamina and a recent test at gym revealed my lungs to be very strong and 100% working.

      I do not have problem with lungs. When i wake up, or even before i fully sleep, something in my throat blocks my nose on the backside when i breathout. So my lungs have to push hard to get the breath out. The breath then seeks its way out through the mouth which leaves me with drooling lips. Its very strange though because even the exit through the mouth is not easy.

      I am really struggling. When i got home yesterday, i started to have severe headache on the backside of my head. I then went into bed at 19:30 and slept till 7AM!

      Is there anyway around the Derby area to share their experience with the Sleep Clinic at Derby. Seems very inefficient to me. When my doctor referred me in March, i got an appointment of 19 Mar. 3 4 days later i received a letter saying my appointment has been cancelled. Went again to the doctor and rebooked for 13th April, this time received letter saying that my appointment has been relocated to 18th May. Rang the sleep clinic and the lady happilly told me that she cant find anything related to me on her computer. She said she will get the booking people to ring me. Its 3rd day now and no call back. £12 billion are being spent on computer systems for NHS and no one is interested in improving the processes and making the system more efficient and speedy.

        I can breath through my nose OK when awake. Its only when i sleep that my nose gets blocked when breathing out. I can feel some blockage in my throat even when i am awake. Especially when i eat or drink something. My elder sister is Hypothyroid but my test show that my thyroid function is normal.

          Those appointment cancellations sound like a nightmare. Big chance if will be August then.

          Given the nasal issue, perhaps you would like to consider seeing an ENT as well. With a little camera (perhaps when sedated, to mimic sleep) they can perhaps see why that exhalation issue is happening. Loosing part of the weight may improve it but there could well be something else going on.

            Thanks for the advice james. So it means this is not the usual issue with other OSA sufferes and most OSA sufferers only suffer from inhaling problems.

              You could say that indeed but then again, if you average all OSA sufferers, no one would be anything like that average. Symptoms vary a lot.

              If you get obstructions on exhalations, they may want to change treatment accordingly. For example a BiLevel device as oppose to CPAP may make it easier to exhale. But that would be up to them, based on why they think it's happening.

                Hi James, not been on line for a while, but still having problems with my leaky mask, noisy hose, and everything that goes with CPAP. I hate it, dread going to bed, dread waking up because it's like I've not been asleep and generally am not happy with the whole thing. I am seriously thinking about getting a MAS and was wondering if you could send me the info and prices. I don' fancy the one with that "strap" on as it looks like it might rub your gums. What's the freebie by the way???!!! Happy Easter michelle

                  Loosing weight is just not enough if it is done for once.You may do so and after some time you may gain again.The key factor is to introduce significant changes in the lifestyle.

                    Pulse Oximeter

                    Hi James,
                    I slept last night with a pulse oximeter on my finger. In the morning, it was showing a flashing 7. I just turned it off instead of trying to get some more figures out of it although i am a software engineer but i did not want to disturb the readings in any way. Do you have any idea what this 7 means? It was a Konica Minolta branded meter.

                      We also use the Konika Minolta 300i for our in-home sleep studies so yes, I know what it's saying. The large number is the blood oxygen saturation, between 0 and 100, during sleep tests typically between 60 and 95. 7 is low! If it really was seven, as in blood oxygen, you should be dead instead of typing this but luckily, you're not!

                      The small number is heart rate, BPM.

                      If you did the test with us, you will get fancy graphs and all sent home. If done through the NHS, I hope your software engineer fuelled curiosity will be satisfied with more than a "you have it" or "come back for another test".

                      If you really want to get in there deep, the data is saved in a pretty simple txt file with 3 columns. Timestamp, Oxygen and Heart Rate. You can take it from there and graph it the way you like, if you can get your hands on the file.

                        Thanks for the reply. Unfortunately, i have already handed the device back to the hospital this morning so no chance of grabbing the text file. I know there should have been some more readings than just a flashing 7 but i just switched it off as soon as i woke up. Now its down to the NHS to see what the results are. How much does this meter cost to buy?

                          Around £800 with all the software etc. But you don't really need it often, hence us renting it out at a more attractive £99 per night.

                          There's another thread here somewhere where TF and I spoke about pulse oximetry units and I found a link to an inexpensive one which hooks up via USB. Not sure how accurate it was or useful, but could be need for those wanting to know more, on the cheap.

                            Update

                            Hi guys,
                            An update on my case. I had hospital appointment yesterday. The doctor told me i had 36 interruptions in my sleep per hour on average. Oxygen levels dropping to 91.5 on average. They have given me CPAP. Used it last night. Breathing was not a problem. Getting to sleep was a bit issue. I do not remember waking up during the night. Took the mask off around 5 in morning. But unfortunately feeling a bit worse!

                            Any tips please.

                              Day 2

                              Yesterday, i got my nose bone on left side swollen. This is where the mask sits. The reason might be my tendency to sleep on my stomach.
                              This morning, i have got a mark there along with swelling.
                              I still do not feel any improvements. Feeling sleepy right now.
